仙腸関節炎と脊椎炎

仙腸関節炎と脊椎炎

有病率: 86%1,†

仙腸関節炎 は、片側または両側の仙腸関節の炎症である。2,3

脊椎炎 とは、腱や靭帯の骨への挿入部の炎症で、椎骨の実質の炎症が連続するものである。2,3

どちらの症状も、40歳未満で発症し、運動すると改善するが安静にしていると改善せず、夜間痛を伴うこともある。3

関節外症状

乾癬

有病率: 13%1

自己免疫性皮膚疾患のひとつで、以下の部分に赤く、かゆみのある、鱗屑状の肌の症状が出ることを特徴とする9:

  • 膝
  • 肘
  • 体幹
  • 頭皮

乾癬はaxSpAの症状・徴候である可能性がある。この病型を乾癬を伴うaxSpAとして分類すべきか、体軸性の病変を伴うPsA（体軸性PsA）として分類すべきかは不明である。しかし、この2つの病態には細かな違いがある: 乾癬を持つaxSpAの患者は、体軸性PsAの患者よりも末梢症状が少なく、HLA-B27陽性であることが多い。10

関節外症状

急性前部ぶどう膜炎

有病率: 16%–37%4,5

前部ぶどう膜の非感染性急性炎症。6

症状6:

  • 目の充血と痛み
  • 飛蚊症
  • 羞明
  • 目のかすみ

axSpAにおける急性前部ぶどう膜炎（AAU）は、単発性、前部、片側性のフレアが認められ、最長で3 カ月続くことがある。7 AAUの有病率は罹病期間とともに増加し、AAUの再発や未治療により緑内障、白内障、視力低下を引き起こし、QOLに大きな影響を与える。8

AAU=急性前部ぶどう膜炎。

関節外症状

炎症性腸疾患（IBD)

有病率: 5%1

消化管の慢性炎症がみられることがある。11

症状は以下を含む12:

  • 持続性の下痢
  • 腹痛
  • 直腸出血
  • 体重減少
  • 疲労

IBD＝炎症性腸疾患。

末梢関節

末梢性関節炎

有病率: 15%–58%4,15

主に下肢にみられる末梢関節の関節炎。15 最近の研究では、axSpA患者の65.9％が体軸関節病変の後に末梢関節炎の初発を認めており、これらの患者では継続的なモニタリングが必要である。15

末梢関節

付着部炎

有病率: 34%–74%14

靭帯、腱、または関節包の骨への挿入部の炎症で、踵（アキレス腱または足底腱膜の挿入部）が最も頻繁に冒される最初の場所であり、痛みを引き起こす。13

末梢関節

指趾炎

有病率: 6%–8%4,13

腫脹につながる炎症は、手よりも足に起こることが多いが、最近の研究では、これはSpAの病型によって異なる可能性があるとされている。​​​​​​​13
